NSBS Event Registration and Release Forms:

**Please fill out the application and release form and drop off at the Nate Smith Basketball School Gymnasium located on 819 Greenbag Rd. Morgantown, WV (please note this is the physical address and NOT our postal address).

We are open (Mon. - Fri. 3pm-8pm) and (Sundays 11am-7pm); there is a  drop box for your convenience if you cannot make our business hours. After school lets out, applications submitted after June 7^th can be dropped off from (Mon. – Fri. 9am-5pm).**
